(Performing arts groups) How does a performing arts group change its chairman?
Required documents:
(1) an original application form.
(2) a photocopied ID card of the group’s new chairman.
(3) original clearing or credit documents of new chairman, which are provided by the Joint Credit Information Center (10F., No.2, Sec. 1, Chongqing S. Rd., Zhongzheng Dist., Taipei City or 02-2381-3939).
(4) 1 photos with 2 x 2 inches in size of new chairman.
(5) the original registration certificate.
(6) the original basic data form.
(7) certificate fee NT$500.
