(Performing arts groups) How does a performing arts group change its chairman?

Required documents: (1) an original application form; (2) a basic data form in duplicate (both original); (3) a photocopied ID card of the group’s new chairman; (4) original clearing or credit documents of new chairman, which are provided by the Joint Credit Information Center (10F., No.2, Sec. 1, Chongqing S. Rd., Zhongzheng Dist., Taipei City or 02-2381-3939); (5) 2 photos with 2 x 2 inches in size of new chairman; (6) an original letter of commitment signed by new chairman; (7) a transfer agreement signed by the group’s original chairman; (8) the original registration certificate; (9) the original basic data form; and (10) certificate fee NT$500.
